ANESTHESIOLOGY & PAIN MANAGEMENT
The goal of the CMC professional staff of anesthesiologists is to eliminate the discomfort and anxiety associated with all forms of care and is available to provide a complete range of services to internationally accepted and accredited standards.

Bariatric Surgery, commonly known as weight-loss surgery, is indicated for patients with a Body Mass Index (BMI = kg/m2) above 40 or for patients with a BMI ranging between 35 and 40 and having related health issues. Moreover, diabetic patients with a BMI greater than 30 can also chose bariatric surgery as a treatment approach.
Bariatric Surgery at CMC is performed laparoscopically (a minimally invasive method) and can also be done without any evident scar through the technique: the Single Site Trans-Umbilical Laparoscopy.
The types of Bariatric Surgery provided at CMC are:
1.Gastric Banding: a silicone, hollow and inflatable ring is positioned around the entry of the stomach. The surgeon modifies the diameter of the entry of food to the stomach according to the weight-loss and eating capabilities of the patient.
2.Sleeve Gastrectomy: a laparoscopic surgery where 80% of the stomach is removed. This results in a small gastric pouch and reduces the volume of food intake and hunger.
3.Roux-in-Y Gastric Bypass: a laparoscopic procedure where the stomach is divided in two parts:
a. A larger part that remains closed and is excluded from the digestive tract.
b. A smaller and functional gastric pouch that is connected to the esophagus and receives very small meals. It is then connected to the bowel (bypassing 1.5 to 2 m of the bowel) to re-establish the continuity of the digestive circuit.
The result of the Roux-in-Y Gastric Bypass Surgery is double: limited food intake and lower calories absorption because of the partial bowel exclusion. Vitamin supplements will have to be prescribed to the patient to avoid deficiencies.
Bariatric Surgery is the only validated treatment for severe/morbid obesity. It is performed at CMC exclusively under laparoscopy and can be done with no visible scar. Complication rate is not significant with patients being carefully selected and operated on by an experienced surgical team as it is the case at CMC.

The emergency department at CMC is a state-of-the-art 8 beds department which provided compassionate and dignified medical care to all people seeking emergency medical attention. The department is fully equipped and staffed with board-certified physicians and registered nurses specifically trained to handle all kind of emergency cases 24/7 from minor injuries to the most complex medical emergencies. Our staff is trained to diagnose and treat patients in fast and efficient manner.

Clemenceau Medical Center (CMC) has recently opened a new Department of Radiation Oncology to deliver comprehensive, state-of-the-art cancer care with a focus on delivering highly targeted radiation that limits exposure to normal tissue. Clemenceau Medical Center has invested in state-of-the-art radiation technology, equipment, and systems. CMC installed a True Beam© STx using Brainlab© Exactrac© robotic couch which is a highly sophisticated system designed to deliver powerful radiation with pinpoint accuracy, speed and precision to treat cancer tumors. The high energy x-rays are shaped to the tumor in order to shrink and kill the cancer cells. The incredible accuracy of the beams reduces the amount of healthy tissue exposed to radiation. This linear accelerator can also deliver the radiation more efficiently, reducing the number of treatments needed. The most advanced treatment planning and information management software (Aria®) is also being used by the Department of Radiation Oncology at CMC to provide an integrated network for fast, efficient radiation planning and treatment that minimizes waiting times and provide a safe and focalized radiation treatment to our patients. The newest technologies in radiation oncology are available at CMC: Stereotactic Body Radiation Therapy (SBRT), Stereotactic Radiosurgery (SRS), Rapid Arc ®, and Respiration Gated radiation therapies. The beam of radiation produced by the True Beam® STx is very focalized because the MultiLeaf Collimators are high definition.
The treatment couch for the radiotherapy treatment is the BrainLab® Exactrac® robotic couch that allows 6 degree of freedom movement including rotation movement. This is the first BrainLab® ExacTrac® that has been installed in Lebanon and the Middle East. This robotic couch allows the Linear Accelerator to treat a moving target with a focalized beam of radiation with the utmost precision.
To calculate the dosimetry for our patients, we acquired the Acuros® software and the Monte Carlo calculation system which will enable us to provide our treatment plan with a very accurate dosimetry. The contouring software can do elastic fusion between different imaging modalities to enable us to contour the target with extreme precision. The software can also do a biological optimization to make sure that the dose of radiation prescribed has been delivered accurately.

History of Robotic Surgery:
The daVinci Robot® is manufactured by Intuitive surgical® in the United States. It has been exponentially adopted in medical centers both in the United States and Europe.
It is used in multiple disciplines ranging from Urology, to general surgery, gynecologic, cardiothoracic and pediatric surgery.
Robotic surgery has now changed patient care and established itself as one of the most important and promising breakthroughs in medical innovation.
What is the daVinci Robot?
The daVinci Robot is a system that is attached to the laparoscopic trocars giving the possibility to the surgeon to perform a laparoscopic surgery with multiple advantages.
It is comprised of 3 main elements:
1. The patient bed side cart which contains the 3 or 4 arms that will be attached to the patient, a camera and 2 or 3 micro instruments according to the surgery performed.
2. The surgeon console where the surgeon sits and operates the robotic arms, he has a 3 dimensions High Definition view of his surgical field allowing him to perform precise and complex movements.
3. A tower that has the main computer and ancillary equipments needed like an energy source, the computer mainframe, cable connections, etc.
What are the Advantages of Robotic Surgery?
Robotic surgery is an advanced form of laparoscopic surgery.
The advantages can be divided into advantages over the traditional “open” surgery: translating into less pain, better cosmesis ( small 8mm incisions instead of one large 8-10 cm incision) less blood loss and risk of blood transfusions and short hospital stays (less than 24 hrs compared to the 3 or 4 days of open surgery).
Advantages over laparoscopic surgery include: better ergonomics, more precise movements, endo-wrist movements and tremor reduction, improved 3D HD endoscopic view; all allowing the surgeon to perform the most complex surgeries without compromising on the minimally invasive approach.
Robotics in General Surgery, Dr. Claude Tayar:
The first Robot assisted procedure in General Surgery was performed at CMC Thursday 24th of July 2012. This constitutes a major breakthrough in surgical practice in both Lebanon and the region. A team of international experts recruited as full-timers at CMC, led by Professor Claude Tayar, performed since a series of several patients with different diseases. Pr Tayar started the Robotic Surgery program in his field 10 years ago in the University Hospitals of Paris and became a European expert and proctor for this new technology.
Major advantages of this technology include a real 3D HD vision of the abdominal cavity with articulated instruments and sophisticated software enabling very precise movements in performing laparoscopic surgery. The Robot allows more surgeons to perform procedures with a minimally invasive approach and permits to experienced laparoscopic surgeons to do procedures that are done otherwise by a classical open fashion. This technology reduces by these means blood loss, pain and complications of surgery.
Applications of Robotic Surgery include: Gastric surgery (cancer, reflux, Gastric Bypass, Band, Sleeve gastrectomy), Liver and bile duct, Pancreatic Resections, Intestines (small bowel, colon, and rectum) and Hernia.

Robotic assistance in Gynecologic surgery, Dr. Karim Nawfal:
When a women needs to undergo surgery multiple parameters have to be taken in consideration. First is the most optimal treatment of the disease, but also the surgical outcomes, the recovery and the final cosmetic result.
Now with robotic surgery, the most complex gynecologic surgeries can be performed with very small (8 mm) incisions, allowing the surgeon to perform a more precise procedure and providing the patient with improved surgical outcomes in form of short hospital stay, less bleeding, less pain, better healing and also leaving very little to no surgical scars.
When medically indicated removal of the uterus (hysterectomy); removal of uterine fibroids (myomectomy); surgeries for endometriosis or the ovaries, certain types of gynecologic cancers and surgeries for pelvic organ prolapse can now all be performed using this newest technology.

The daVinci surgical robotic system made it possible to cure most urologic diseases and cancers without compromising the quality of life for patients.
Robotic prostate surgery enables surgical precision beyond the limits of the human hand. The 3-D view allows surgeons to clearly identify the delicate nerves and blood vessels around the prostate. Today, most prostate cancer patients (70% of cases in the United States) are operated successfully using robotic surgery while maintaining normal bodily functions such as urinary control and erectile function.
In many cases, kidney tumors can be removed without damaging the remaining healthy portion of the kidney (partial nephrectomy) thanks to these robotic procedures.
Many other urologic procedures can be performed safely and effectively with Robotic assistance such as: adrenalectomy, total nephrectomy, pyeloplasty, surgical treatment of urothelial tumors (bladder and upper urinary tract), treatment of uro-genital prolapse (sacro-colpo-pexy), lymph node dissection, and some cases of complicated urinary stones.
The Rheumatology Service at CMC offers consultative diagnostic and therapeutic services for outpatients and inpatients with joint disorders and soft tissue injuries.
What is Rheumatology?
Rheumatology is the treatment of joint and muscle diseases and the systemic (diffuse and/or generalized) inflammation in the human body. Rheumatism, or rheumatic diseases, can involve any organ including but not limited to the joints, muscles, blood vessels, skin, kidneys, liver, eyes, and brain. It usually starts with mild articular pain and stiffness at night and in the morning hours. When left untreated or when treated by non-rheumatologists, it can lead to permanent damage of the articulations and involvement of the deeper organs. In severe cases, rheumatism can lead to the inability to walk and sometimes even death.
The etiology is not clear, but the most agreed theory is that a dysfunction in the immune system turns against the organs, rather than protecting the body from viruses and bacteria, and causes inflammation, pain and damage.
How is Rheumatism treated?
The treatment depends on the stage the patient has reached. At first the treatment consists of oral medications that modulate the immune system. However, patients should be attentive to not accept a treatment based solely on pain medications and simple anti-inflammatories, as they mask the pain and do not prevent joint destruction and cause permanent damage.
The more advanced cases are treated with a combination of more than one medication.
Patients whose conditions progress rapidly and cause significant swelling and pain might need self-injectable or infusible medications beside the pills. The self-injectable medications are engineered to be self administered, while the infusible medications are administered at the hospital in the same-day unit, which means the patient comes to the hospital and leaves a few hours later; there is no need to stay overnight. Until the patients’ body gets accustomed to the treatment, it is recommended that patients do not work the day of infusion as the treatment can cause fatigue. Those infusions are done every, 4, 6, 8, 16, or 24 weeks depending on the medication, and the Rheumatologist will explain the medication’s instructions to the patients.
During a patient’s visit to the Rheumatology clinic at CMC, the rheumatologist could perform an injection of a joint if it is inflamed and not responding to the treatment. It is usually done to prevent the inflammation from causing joint destruction. The medications (corticosteroid + anesthetic) reach the inflamed tissue directly and thus the injection is highly effective. The patient can expect pain and a burning sensation after the injection because the volume of medications used presses on the muscle and nerve fibers. This usually resolves within 24 hours and the improvement starts happening at this time.
